Many of our forms can be accessed and submitted using MyUI (students) or Employee Self Service (faculty/staff).
University Billing offers a variety of services that require written consent or documentation. For your convenience, we are providing access to our forms from this centralized location on our website. Simply follow the submission instructions provided on each form.

Agreements
Student Agreement-UI Billing and Payment Terms and Conditions
Printable/downloadable copy of the Student Billing Agreement for parent/guardian signature when student is under 18 years of age at the beginning of the semester.
Authorization Forms
Authorization to Apply Excess Financial Aid Funds to Miscellaneous Charges
Form authorizing the use of financial aid refunds towards current charges billed.
Authorization to Grant Charging Privileges to Minor Student (Under 18 Charging Consent Form)
Parent/Guardian authorization to allow student under the age of 18 to use ID card charging privileges.
Automatic Withdrawal Authorization
Form authorizing the automatic withdrawal of funds from a checking or savings account to pay U-Bill. (Only for individuals who cannot use MyUI or Employee Self Service to setup automatic payments.)
Parent PLUS Loan Direct Deposit Authorization
For individuals who DO NOT have a MyUI guest account. (Individuals with guest accounts can enter their direct deposit refund account on MyUI).
Billing Fee Appeals
Late Payment Fee Appeals - Webform to request a waiver of late payment fee (no printing required)
Deferred Payment Fee Appeals - Webform to request a waiver of deferred payment fee (no printing required)
Outside Scholarships
Scholarship Check Submission Form (Academic Year 2025-2026)
Required form for private donor organizations submitting scholarship checks to provide student and scholarship detailed information.

Electronic Billing Exemption Request
Request an exemption from electronic presentment of U-Bill (electronic billing is mandatory for all students and employees).
Request for Third Party Billing
Students who receive support from a third party sponsor are required to complete this form and submit it to University Billing.
